The website of the Office of Chief Coroner has been updated and is now available for use —
The transition from the old site to the new meant many recent Preventing Future Death reports were delayed in their publication, but they are now available online, along with all the historic reports which have migrated across as well. For many of the reports of most direct relevance to this blog, see my PFD resources page.

I regularly read this website, I regard the PFDs as a mine of information about tragic events, many of which went awry in circumstances where there are obvious lessons to be learned. I would argue many professional leads in most public sector organisations touched by the interface between policing, mental health and criminal justice should be regularly surveying this site and I look forward to seeing what can be learned from the last few months’ PFDs, now they are available.
At the heart of each case is the death of someone who is missed by their family and friends.
